Fatalities happen because of numerous factors - many of which are not well understood or addressed by most organizations. Organizations with fatality risk blind spots can operate for many years without a fatality – only to be caught off guard by a tragic incident. After reviewing hundreds of fatalities and near-fatal events, we have distilled the common factors down to six that are always in play prior to a fatal event. Session participants will become familiar with each of these factors and how they evolve and align to create fatality risk and ultimately fatalities.
