Do You Have a Risk Blind Spot?

Fatalities happen because of numerous factors - many of which are not well understood or addressed by most organizations. Organizations with fatality risk blind spots can operate for many years without a fatality, only to be caught off guard by a tragic incident. After reviewing hundreds of fatalities and near-fatal events, we have distilled the common factors down to six that are always in play prior to a fatal event. Session participants will become familiar with each of these factors and how they evolve and align to create fatality risk and ultimately fatalities.
Bio:
Peter T. Susca, M.S., is a principal at OpX Safety and has over 40 years of environment, health and safety, business leadership and process improvement expertise. He has served in various EHS technical and senior management positions in large multinational corporations. He has developed EHS management systems, rating systems, auditing and auditor certification programs, risk assessment pro-cesses, educational management systems, executive EHS development programs, quality and EHS systems integration, incident investigation and high-risk industry fatality prevention programs for a wide variety of cli-ents.
